We provide these forms so you may fill them out before your first visit. You are welcome to fill them out electronically or by hand.
Review and complete the following intake forms, as your therapist will go over them with you at your first session:
– Therapist-Client Agreement (Required)
– Financial Agreement (Required)
– Telehealth Consent (Required if participating in telehealth sessions)
Your clinician will let you know which of the following forms to complete:
– Confidential Intake – Long
– Confidential Intake – Short
Access the client portal. You will be able to fill out forms, schedule, and manage your appointments. Not all of our therapists allow online appointment setting, so if your clinician does not have this feature enabled, continue to schedule appointments as usual. Using the portal requires an invitation from your therapist. Please reach out to your clinician if you would like access to this service.
TPC attempts to make counseling affordable by joining insurance panels and by using an adjustable fee scale for those who do not have insurance. However, if an uninsured client is unable to afford the minimum amount on the fee scale, TPC has two funds which offer short-term assistance to those who qualify.
